Chronic wounds associated with diabetes, venous insufficiency, pressure, and neuropathy remain challenging because persistent inflammation, impaired cellular signaling, and extracellular-matrix dysfunction can disrupt normal tissue repair. This Innovation Theatre will examine high-purity, native phosphorylated Type-I collagen as a biologically active extracellular matrix, emphasizing that not all collagen-based products function merely as wound dressings. Collagen performance depends on its purity, native fibrillar architecture, processing, and biochemical properties. Particular attention will be given to the diabetic wound microenvironment, where excess glucose and glycation can impair cellular function, matrix remodeling, angiogenesis, and tissue regeneration. The potential of native Type-I collagen to interact with and sequester excess glucose locally, thereby potentially reducing its adverse effects while providing the matrix favorable for cellular attachment, migration, and repair, will be explored. The session will also highlight phosphorylation of Type-I collagen and its potential influence on cell signaling, angiogenesis, and regenerative healing. Integrating molecular mechanisms with emerging clinical evidence, the presentation will illustrate how appropriately engineered Type-I collagen matrices may extend beyond passive wound coverage to actively support the biological processes required for healing chronic diabetic ulcers. Sponsored by Encoll Corp.

Infection remains one of the most common complications of chronic wounds, which can remain open for months and create ongoing opportunities for microbial contamination and infection. At the same time, antimicrobial resistance is a growing global threat, a significant proportion of antibiotic use is considered unnecessary or inappropriate, and the pipeline for new antibiotics continues to shrink. Reinforcing the importance of antimicrobial stewardship. So, what role can topical wound care play? Pure hypochlorous acid (pHA) provides broad-spectrum antimicrobial activity with no known microbial resistance, offering a stewardship-minded approach to managing wound bioburden. Traditionally used as a solution for cleansing and wound bed preparation, a new sustained gel format raises an important question: What more could pHA do beyond cleansing? This session will explore emerging clinical evidence for Vashe® Antimicrobial Wound Gel, including its potential to extend contact time and support antimicrobial activity, wound acidification, debridement, and healing. Through clinical cases spanning complex adult wounds to the first reported uses in pediatric and neonatal patients, attendees will discover how a trusted molecule in a new format may offer a simple solution to increasingly complex wound care challenges. Chronic, nonhealing diabetic foot ulcers do not follow the normal, orderly progression of events seen in healthy acute wounds. Many theories have been proposed to explain the observed pathophysiologic state of these wounds. Traditionally, chronic DFUs have been considered to be trapped in a persistent inflammatory state, with impaired transition toward pro-resolution macrophage phenotypes. Graphically, this is displayed as a prolongation of the bell shaped curve of “inflammation” following hemostasis, and preceding healing. The cells within and at the edge of chronic wounds have been described as senescent, which aligns with the observation that aggressive debridement can support healing. One inconsistency was observed with the use of fibroblasts and keratinocytes that were made replicatively senescent via exposure to radiation. Spraying these living, senescent cells onto the surface of a chronic VLU promoted much better rates of healing (Kirsner 2016). How can senescent cells be the answer if cellular senescence is the problem? More recent single-cell analyses of human DFUs have challenged the conventional M1/M2 paradigm, identifying distinct cellular states associated with healing and nonhealing wounds, including enrichment of inflammatory macrophage populations in healers (Theocharidis et al., 2022). This session will reconsider the conventional view of inflammation in DFUs, explore how the nature and timing of inflammatory responses influence healing, and discuss novel therapeutic approaches that harness, rather than simply suppress, inflammation. Sponsored by Flen Health.

Autologous therapies are transforming the management of chronic and complex wounds, but selecting the right product for the right patient requires a clear understanding of product differences, clinical evidence, and practical implementation. Through expert discussion and real-world cases, this session will explore patient selection, timing of therapy, blood draw considerations, clinical guidelines, and the nuances of integrating autologous therapies into comprehensive wound care treatment plans. Faculty will also address workflow optimization and reimbursement strategies to help clinicians confidently incorporate these advanced therapies into everyday practice.

Unfortunately, many wound and ulcer management professionals, coders, and billers have developed poor reimbursement practices that are leading to a substantial number of denied claims and repayments. This session will provide you with a rare opportunity to 1) gain insights from consultants who collaborate daily with stakeholders who face these challenges and 2) appreciate the importance of recognizing and promptly correcting problematic reimbursement habits.

Wound and ulcer management professionals, coders, and billers have complete control over their reimbursement processes. Although it is not possible to prevent pre-payment and post-payment reviews and audits, it is possible to cultivate effective habits that ensure appropriate reimbursement and retention of payments after audits. Because this session will feature practical advice from experienced consultants working in this field, attendees will have a unique opportunity to identify key reimbursement habits they should adopt immediately when they return to their roles.

Attendees will gain practical guidance to balance efficacy, safety, and stewardship in the management of wound infections.
