Measurable residual disease (MRD) has rapidly evolved from a prognostic marker to one of the most influential tools in acute leukemia management. Modern MRD testing uses highly sensitive techniques such as: 

  • Multiparameter flow cytometry 

  • Next-generation sequencing (NGS) 

  • Molecular testing platforms 

These technologies can detect one leukemia cell among thousands—or even millions—of normal cells, providing clinicians with a much more precise assessment of treatment response. 

Why Is MRD Becoming So Important?

MRD adds another layer of information. 

Research has consistently shown that patients who achieve MRD negativity generally experience lower relapse rates and improved survival outcomes compared with patients who remain MRD positive after treatment.

  • Is the current treatment working? 

  • Does this patient need a stem cell transplant? 

  • Should therapy be intensified or modified? 

  • Is a patient at higher risk of relapse? 

Rather than serving solely as a prognostic marker, MRD is becoming a practical decision-making tool that may help clinicians personalize treatment strategies. 

How Is MRD Changing Treatment Decisions in ALL? 

Some of the most significant advances in MRD-guided care have occurred in acute lymphoblastic leukemia (ALL). 

Recent studies have demonstrated that MRD status can influence treatment selection, timing of intervention, and decisions regarding allogeneic stem cell transplantation. 

The growing use of immunotherapies such as blinatumomab has further elevated the role of MRD monitoring. Clinicians are increasingly using MRD assessments to identify patients who may benefit from targeted intervention before overt relapse occurs. 

In addition, MRD is playing an important role in evaluating chemotherapy-free treatment approaches and helping clinicians determine which patients may safely avoid more intensive therapies.

Could MRD Replace Traditional Response Assessment? 

Not entirely. 

While MRD is becoming an increasingly valuable tool, it is not intended to replace traditional clinical evaluation, bone marrow assessment, molecular testing, or physician judgment. 

Several challenges remain, including: 

  • Variability in testing methodologies 

  • Differences in assay sensitivity 

  • Timing of assessments 

  • Standardization of reporting 

  • Interpretation of low-level positivity 

Ongoing efforts are focused on improving consistency across testing platforms and establishing evidence-based guidelines for incorporating MRD into routine practice.

MRD is increasingly transforming the management of acute leukemias by providing a more sensitive measure of treatment response and relapse risk than traditional assessments alone.

While questions regarding standardization and implementation remain, MRD has become one of the most important emerging tools in precision leukemia care and is likely to play an even larger role in future treatment paradigms.
