Wound Care Meets Vascular: Closing the Gap from Wound to Revascularization and Back (SVS & SVN Collaboration)

Wound clinicians are often the first to spot chronic limb-threatening ischemia and the last line of surveillance after a procedure, but too many patients still fall through the gap between the wound clinic and the vascular team. In this joint SVN/SVS session, an expert panel follows a single patient’s journey from first warning signs through revascularization and back to long-term limb preservation. Faculty will cover how to recognize when a wound isn’t healing because of poor blood flow, which tests to order (and why a “normal” ABI can be misleading), how revascularization decisions are made, and what the wound team should watch for afterward. The session also addresses care beyond the hospital, including offloading, nutrition, smoking cessation, medication management, and making sure follow-up imaging actually happens. Attendees will work through real-world cases with the panel and leave with practical steps to build a faster, more reliable referral pathway between wound care and vascular specialists.
