Blood transfusion has shaped battlefield survival for more than a century, evolving from frontline whole blood use in WWII to the modern return of low-titer O whole blood in both military and civilian systems. As global conflicts, large-scale incidents, and austere deployments continue to challenge evacuation timelines, medics must be prepared to initiate resuscitation long before reaching a hospital. Understanding this historical progression—from early whole blood to component therapy and now back to whole blood—helps explain why donor-based and autologous transfusion techniques remain essential.
This session focuses on how autologous whole blood transfusion can serve as an effective training method to prepare medics for the realities of MASCAL events, wilderness and remote medicine, and prolonged field care. By teaching the principles of walking blood banks, donor screening, emergency collection, and transfusion workflows, educators can build realistic, competency-based scenarios that mirror resource-limited environments. Participants will learn how to safely integrate autologous transfusion concepts into EMS and military-medic curricula and why these skills are increasingly important for modern prehospital care.
