Jenny Rieker, CP, CCEMTP, serves as Battalion Chief of Mobile Integrated Health for the Mehlville Fire Protection District, where she leads the district’s Mobile Integrated Health Division and supports programs including High Utilizer Engagement, Fall Referral, and Harm Reduction initiatives. With more than 20 years in prehospital care—and over a decade focused on Mobile Integrated Healthcare (MIH)—Jenny has worked across both hospital and fire-based EMS systems. As one of Missouri’s early community paramedics, she contributed to the development of the state’s first MIH program and remains actively involved in advancing MIH efforts statewide. She is passionate about collaborative care models that improve patient outcomes and regularly presents at regional and national EMS conferences.

The Hidden Workforce Behind 911: How MIH Prevents Emergencies Before They Happen

Mobile integrated healthcare (MIH) teams are often viewed as “post-crisis responders,” but in reality they are one of the most effective preventive medicine tools inside the modern fire/EMS system. This session reveals the hidden, upstream work that MIH providers do every day—identifying medical, behavioral, and environmental risks long before they trigger a 9-1-1 call or emergency department visit.

Through real-world examples, attendees will learn how MIH identifies early signs of infection, decompensation, medication issues, fall hazards, social instability, and gaps in chronic disease management. We will discuss how proactive home visits, trending vital signs, in-home labs, health literacy coaching, behavioral health support, and community resource navigation prevent crises, reduce system strain, and significantly improve patient stability.

The session also highlights the impact MIH has on high utilizers, patients with chronic illness, individuals with behavioral health or substance-use challenges, and homebound or socially isolated adults. Attendees will leave with actionable strategies for building preventive systems, including screening tools, follow-up models, interdisciplinary teams, and ways to measure upstream impact.

This presentation is ideal for fire/EMS leaders, clinicians, and decision-makers who want to understand how MIH strengthens the entire 9-1-1 system by preventing emergencies before they ever occur.
