As EMS systems face rising call volumes, deployment challenges, and hospital overcrowding, Mobile Integrated Health and Community Paramedicine programs offer a patient-centered approach to reducing unnecessary ambulance utilization while ensuring optimal patient care. In this session, Wake County EMS shares the origin, structure, and clinical outcomes of its Advanced Practice Paramedic (APP) program. Guided by a mission of prompt, compassionate, and clinically excellent care, the program operates on three core pillars: Respond, Reduce, and Redirect. Attendees will learn how Wake County built a community paramedicine model that supports field crews, addresses social determinants of health, and connects patients with the right care at the right time.
