Drawing on two decades of experience developing and maintaining oncology clinical pathways, this session will examine how provider, payer, and institutional pathway programs have evolved—and how their underlying business models influence their design and performance. Attendees will explore the progress made toward high-quality pathway programs, the structural gaps that remain, and key successes from the past 20 years. The session will also address persistent challenges affecting patients and physicians, including limited cost transparency, missed clinical research opportunities, administrative burden, and costly implementation. Looking ahead, proposed solutions will focus on accelerating the integration of new evidence, strengthening prospective decision support, improving payer alignment, and streamlining prior authorization to reduce delays in care.
