New research from King’s College London indicates that deaths from nitazenes—a class of extremely potent synthetic opioids—are likely being significantly undercounted in the UK, with implications for public health surveillance and intervention design. Nitazenes can be up to 500 times stronger than heroin, and while the UK National Crime Agency (NCA) reported 333 nitazene-linked fatalities in 2024, scientists now suggest the true figure may be about one-third higher because many deaths go undetected in standard postmortem toxicology.

Laboratory experiments revealed that only about 14 % of nitazene present at the time of overdose remains detectable in blood samples by the time traditional pathology testing occurs, due to rapid chemical degradation. When researchers applied statistical modeling to national mortality data, they found evidence of excess drug deaths in areas like Birmingham that could be explained by undiagnosed nitazene involvement.

Experts warn this undercount undermines effective public health strategies and harm-reduction planning, as policymakers and providers may be designing responses based on incomplete data. The findings underscore the need for improved forensic detection methods and heightened awareness of synthetic opioid threats among clinicians, researchers, and harm-reduction stakeholders
