Session Description: The landscape of novel synthetic opioids (NSOs) adapts rapidly to international regulatory interventions. Following China's class-wide scheduling of nitazene (2-benzylbenzimidazole) analogues in July 2025, the illicit drug supply experienced a significant structural shift. As dominant, high-potency nitazenes like metonitazene and protonitazene have declined, a new subclass of NSOs has emerged to fill the market void: colloquially known as "orphines", or scientifically as benzimidazol-2-one derivatives. The primary analogue among these is N-propionitrile chlorphine (aka cychlorphine), an ultra-potent opioid with an in vitro potency estimated to be roughly ten times greater than fentanyl. Through NPS Discovery, the CFSRE's U.S.-based drug early warning system utilizing high-resolution mass spectrometry and advances sophisticated methodologies to analyze forensic data, seized drug samples, and toxicology biospecimens, we have documented this rapid market pivot across North America from late 2025 into early 2026.
Orphine-type novel opioids are increasingly identified, either as the only opioids or as standalone adulterants and co-mingled with fentanyl, directly contributing to severe adverse health events and fatal overdoses. Specifically, cychlorphine has been identified in at least 20 states and linked to more than 140 deaths. This presentation provides critical, timely intelligence on the proliferation, pharmacology, and geographic distribution of orphine-type opioids. Understanding this next wave of the opioid epidemic is vital for deploying proactive public health interventions, optimizing harm reduction strategies, and guiding law enforcement supply-control operations.
