Session Description: The North American drug landscape is dynamic, volatile, and increasingly toxic as drug markets have shifted towards primarily synthetically manufactured substances. Today, traditional drug supplies are commonly substituted or adulterated with other substances and there is a trend towards more potent substances appearing in drug products. Fentanyl remains the most common opioid and primary driver in drug overdose, but it is rarely observed without the presence of other CNS depressants and sedatives. Medetomidine, an alpha-2 agonist more potent than xylazine, continues to proliferate across the U.S., creating worsening adverse effects and increased emergency department admissions. Fentanyl is also increasingly found with local anesthetics (e.g., lidocaine, tetracaine), new benzodiazepines (e.g., bromazolam), and other substances.
The trajectory of emerging drugs and novel psychoactive substances continues down a similar path of potency and toxicity. Drug control actions in the U.S. and around the world continue to shift emerging drug landscapes. This is evident by the recent class-wide control of nitazene analogues in China in 2025 and the beginning proliferation of orphine analogues in the U.S. in 2026. As communities begin to grapple with this new class of potent opioids, there has been a notable resurgence of potent synthetic cannabinoids in the U.S., as well.
Full understanding of the drug supply remains limited due to inconsistency in testing and reporting practices. The true extent to which these emerging drugs and adulterant impact drug use and overdose remains largely unknown. Increasingly potent substances challenge analytical methods in laboratory settings and there are downstream issues caused by not knowing the future slate of drugs consumed. Clinical awareness and response to the presence and evolution of emerging drug supplies remains rooted in better drug testing and interpretation. This session will translate current drug-supply intelligence into practical clinical considerations, including what emerging substances may mean for patient presentation, what common testing gaps can occur, and how to communicate risk and uncertainty with patients and care teams. A brief clinical scenario will be used to illustrate decision points around when to broaden testing, how to interpret results and limitations, and how findings can inform clinical response and harm-reduction counseling.
