Dr. Peter “Rocky” Samuel, MD, MBA, FACEP, is a physician executive and nationally recognized leader in value-based, senior-focused care delivery. He serves as Chief Medical Officer of Centered Care, where he leads national clinical programs bringing care coordination and integrated behavioral health into senior living communities. By leading multidisciplinary teams and leveraging technology, his work focuses on keeping seniors healthier and out of the hospital. He currently practices as a primary care physician, maintaining active frontline clinical engagement.

Dr. Samuel is also the Physician Owner of Bethesda Assisted Living of Plant City, a multi-community assisted living organization in Florida built on a clinically led, family-centered, and evidence-based care model.

Board-certified in Emergency Medicine, Dr. Samuel holds both an MD and MBA from Harvard Medical School and Harvard Business School. His career spans frontline clinical practice and executive leadership, including senior clinical roles at DispatchHealth, MedArrive, and Troupe Health, where he advanced models integrating medical care into home- and community-based settings.

As a physician owner, Dr. Samuel brings a rare combination of bedside experience and strategic leadership. His work focuses on improving outcomes for older adults through proactive care models, interdisciplinary team leadership, and data-driven quality improvement. He is deeply committed to advancing equity, dignity, and innovation in senior living, and to empowering care teams to deliver compassionate, high-reliability care at scale.
