I am a board-certified emergency physician with over 20 years of clinical experience and serve as Chief Medical Officer and Co-Founder of C-Booth Innovations, where I lead clinical strategy and development of diagnostic EKG technologies to improve cardiac care. I have co-authored multiple patents and publications in flexible electronics and EKG innovation. I also serve as an Adjunct Research Associate Professor at the University of Southern California, teaching molecular microbiology, immunology, and epidemiology, and as Chairman of Medical Staff Peer Review at Palomar Health. My experience includes work as a principal investigator in device and pharmaceutical studies, presentations at professional conferences, expert witness service in medical malpractice and criminal cases, and leadership as Emergency Director of a Federal Medical Station during the COVID-19 pandemic.

Garbage In Garbage Out: The Critical Role of Proper EKG Acquisition in Accurate STEMI Identification

EMS professional spend a great deal of time learning to interpret EKGs. This is based upon the fundamental importance of the skill, particularly in accurately assessing for STEMI but also rhythm diagnosis and for such things as cardiotoxicity from toxins.

In all of these applications, the assumption is that the EKG itself has been obtained in the prescribed manner, which includes skin prep and proper lead placement. The failure to provide good skin prep is often a reason for artifact; this is sometimes apparent. However lead placement and patient positioning are other critical factors that are not as easily seen on the EKG and, if done improperly can cause a false negative or a false positive STEMI.

This talk provides fundamental information on skin prep (alcohol wipes and even a quick scrape with electrode holder are insufficient!) and lead placement that many assume they know but come to find out that their skills could improve.

Michael Levy, MD, FAEMS, FACP, is the Chief Medical Officer for Anchorage Areawide EMS and Medical Director for multiple EMS agencies across Alaska, including the Anchorage Fire Department and the Kenai Peninsula Borough. Based in Anchorage, he is an Affiliate Associate Professor with the University of Alaska and has more than three decades of experience as an emergency medicine and EMS physician. Dr. Levy earned his BA from the University of Colorado Boulder and his MD from Northwestern University before completing a combined Internal Medicine–Emergency Medicine residency at McGaw Medical Center. A past president of NAEMSP, he is an internationally recognized speaker on resuscitation, EMS systems, rural EMS, and high-performance CPR, and has held key leadership roles in national guideline development and EMS quality improvement.
