Carolyn Clevenger

Carolyn Clevenger is the Founder of the Integrated Memory Care model. The IMC is a nurse-led primary care practice for people living with dementia with an outpatient clinic and senior living community practice across metro Atlanta. The new model of care has been recognized as a Patient-Centered Medical Home, an Age Friendly Health System participant, a Best Program for Caregiving, a Best Practice Exemplar for patient- and family-centered care, and an Edge Runner. The practices boasts the extraordinarily high patient experience scores and significantly reduces the risk of hospitalizations for people living with dementia.

Dr. Clevenger's research portfolio includes psychoeducational programs to improve mastery among dementia family caregivers and the geriatric healthcare workforce. She earned her BSN from West Virginia University, her MSN from Emory University, and her DNP from the Medical College of Georgia. She completed her postdoctoral fellowship at the Department of Veterans’ Affairs.

Planning for Progression: Long-Term Care Strategies in Dementia

Dementia is a progressive condition that requires proactive, longitudinal care planning across clinical, psychosocial, and systems-based domains. This session will equip clinicians with practical strategies to anticipate disease progression, align care with patient and caregiver goals, and coordinate services across the continuum of care. Through a case-informed approach, participants will explore best practices in early and ongoing care planning, including advance care discussions, risk mitigation, behavioral symptom management, caregiver support, and transitions to higher levels of care. Emphasis will be placed on interdisciplinary collaboration among psychiatry, primary care, neurology, and allied health professionals to optimize outcomes and quality of life for patients living with dementia.
