A Bolder Model: How Should We Be Training the Next Generation of Therapists?

There seems to be something wrong with our clinical training approach. One of the most replicated findings in psychotherapy research is that therapists, unlike surgeons, usually don’t improve with practice: their treatment outcomes after 30 years are about the same on average as when they started. This Great Conversation will focus on how we might use a science-based approach to train the next generation of psychotherapists so that they do get better with practice.

STEPHEN GILLIGAN, Ph.D., is a psychologist in Encinitas, CA. He was one of the original NLP students at UC Santa Cruz, where Milton Erickson and Gregory Bateson were his mentors. After receiving his psychology doctorate from Stanford University, he became one of the premier teachers and practitioners of Ericksonian hypnotherapy. This work evolved into his original approaches of Self-Relations and Generative Self and was further developed with Robert Dilts into Generative Coaching. In response to the unprecedented meta-crisis we are currently facing, all this work is now woven into a Creative Mind meta-structure that honors and seeks to realize the interconnection of all life—friends and enemies, nature and humans, spirit and matter.

He has taught in many different cultures and countries over the past 30 years and has published extensively. His numerous books include The Hero’s Journey: A Voyage of Self-Discovery (co-authored with Robert Dilts), the classic Therapeutic Trances, The Courage to Love, The Legacy of Erickson, Walking in Two Worlds (with D. Simon), Generative Trance: The Experience of Creative Flow, four volumes of Generative Coaching, and his ongoing six-volume series on Creative Mind.

Feedback Informed Treatment: Improving the Outcome of Psychotherapy 65% without Hardly Trying

What can mental health professionals do to enhance their performance? Available evidence makes clear that attending a typical continuing education workshop, specializing in the treatment of a particular problem, or learning a new treatment model does little to improve effectiveness. In fact, studies to date indicate clinical effectiveness actually declines with time and experience in the field. The key to improved performance is engaging in deliberate practice. At this workshop, the latest research on deliberate practice will be translated into concrete steps all clinicians can immediately apply in their efforts to achieve better results.

Michael Munion

Michael Munion has been a practicing clinician in Arizona, USA for 43 years. He is co-author of Milton Erickson, and co-editor of What is Psychotherapy? Contemporary Perspectives. He teaches nationally and internationally. Most recently he has been involved in The Pain Project which aims to bring effective non-pharmaceutical interventions to individuals suffering chronic pain. He was named to a panel that developed best practice guidelines for the treatment of Co-occurring Mental Health and Substance Use Disorders, and was a member of Arizona’s delegation to the National Policy Academy on the Treatment of Co-occurring Disorders.

It’s not a pretty picture. Available evidence indicates that the effectiveness of psychotherapy has not improved despite 100 years of theorizing and research. What would help? Not learning a new model of therapy or the “latest” so-called “evidence-based” treatment approach. And no, not attending another CEU event or sorting through that stack of research journals by your desk. A simple, valid, and reliable alternative exists for maximizing the effectiveness and efficiency of treatment based on using ongoing feedback to empirically tailor services to the individual client needs and characteristics. Research from multiple randomized clinical trials documents that this simple, transtheoretical approach as much as doubles the effectiveness of treatment while simultaneously reducing costs, drop-out rates and deterioration.

Bob Bertolino, Ph.D., is Professor of Rehabilitation Counseling at Maryville University and Sr. Clinical Advisor at Youth In Need, Inc. in Missouri. He has taught over 600 workshops throughout the United States and 11 countries and authored or co-authored 15 books including Effective Counseling and Psychotherapy: An Evidence-Based Approach, The Therapist’s Notebook on Positive Psychology: Activities, Exercises, and Handouts, and the forthcoming, The Therapist’s Notebook for Supervision and Training: Activities and Exercises to Improve Effectiveness.
