This session aims to explore the critical role of an interprofessional approach in post-acute care and rehabilitation for wound patients by focusing on the multifaceted needs of individuals at risk of and with chronic wounds. This session will highlight the collaborative roles of PTs, OTs, STs, and other healthcare professionals in delivering a comprehensive and effective plan of care. We will delve into the latest research and tech to improve critical aspects of care, such as mobility, positioning, specialty surfaces, contracture management, skin health, gait training, and off-loading. Attendees will discover how the interplay of off-loading, mobility, and exercise are all essential components for healing for most chronic wound types. Further, modality use will be explored as catalyst to activate healing. Lastly, this session will discuss the role of the rehab team in understanding health literacy to influence and improve patient engagement, the primary proxy for a successful plan of care.
