William Heuser, PharmD, BCCCP, BCEMP, BCNSP, MS, EMT-P, FP-C is a clinical critical care pharmacist, course director/clinical professor of pharmacology, clinical toxicologist and certified flight paramedic/firefighter. William has presented at several national conferences across the United States and has extensive research experience in resuscitative medicine with several publications in peer-reviewed journals. As a clinical toxicologist at the Feinstein Institute for Medical Research at Northwell Health, his research focuses on the acute resuscitation of critically ill and dying patients. A current focus of his research focuses on the development of novel pharmacological cocktails coupled with emergency cardiopulmonary bypass to save the lives of patients who fail traditional advanced cardiovascular life support (ACLS) treatment.

Ketamine on Trial: The Case for and Against Prehospital Sedation

This session puts one of EMS’s most controversial medications in the hot seat. In this high-stakes, courtroom-style debate, a paramedic-attorney takes the prosecution’s side—questioning the ethics, safety, and legality of field ketamine use—while a clinical pharmacist–paramedic mounts the defense, armed with evidence, pharmacology, and real-world experience. Together, they’ll examine the data, dissect the lawsuits, and challenge the assumptions surrounding ketamine for undifferentiated agitation, behavioral emergencies, and rapid sedation in the field. Was ketamine used appropriately, or did we cross a line? Where does clinical judgment end and legal liability begin? With expert testimony, cross-examination, and audience participation, this session blends science, law, and clinical practice in a fast-paced, unforgettable format. Attendees will leave with a deeper understanding of both the pharmacologic and medicolegal implications of ketamine administration—and the confidence to defend (or reconsider) their decisions when the next agitated patient tests the limits of prehospital medicine.

Eric Jaeger, JD, EMS I/C, NRP is an attorney, RSI paramedic and EMS educator. He works at Exeter Hospital in Exeter, NH as the EMS educator and as an RSI paramedic on the hospital’s ALS Paramedic Intercept team and in the emergency department. Eric lectures extensively on EMS topics. He has served on the State of NH EMS Protocol Committee for over 15 years. He holds a BS, Computer Science from MIT and a law degree from Boston College Law School from which he graduated summa cum laude. Publications: “Know the Differences between Seizures, Epilepsy & Mimics,” JEMS, 12/2012, and “Prehospital Care of Neurologic Emergencies, Ch. 5 Spinal Cord Injury,” Cambridge University Press, 08/2014.

Peter M. Antevy, MD is a Pediatric Emergency Medicine (EM) physician practicing in-hospital emergency medical care at Joe DiMaggio Children’s Hospital, a level I trauma and tertiary care center in South Florida. Board-certified in pediatrics, emergency medicine, and the complex subspecialty of EMS, he is also the Founder and Chief Medical Officer of Handtevy, and he serves as the Medical Director for Davie Fire-Rescue, Coral Springs-Parkland Fire Rescue, Southwest Ranches Fire Rescue and United Medical Transport, all in South Florida. Dr. Antevy also serves as Associate Medical Director for several other agencies, including Palm Beach County and Brevard County, Florida. He is also the longstanding Medical Director for two highly-regarded paramedic training programs as well as several mobile integrated healthcare (MIHC) programs in greater Broward County, Florida.

Mark Piehl is a pediatric critical care physician at WakeMed in Raleigh, NC, and a Medical Director with WakeMed Mobile Critical Care transport. He is also Founder and Chief Medical Officer of 410 Medical, a company developing technology to improve resuscitation in shock, sepsis, and trauma. Mark teaches frequently on the pre-hospital management of trauma and critical illness, has faculty appointments at the University of North Carolina and Duke University, and has numerous Department of Defense grants funding the development of technology to improve trauma care. He is also a founding member of the SPARC Academy and the North Carolina Prehospital Regional Blood Coalition.

Ventricular Tachycardia Storm: Controversies in Resuscitation 

Ventricular tachycardia remains one of the most lethal rhythms encountered in prehospital medicine, yet management extends far beyond repeated defibrillation and standard ACLS medications. This masterclass provides a comprehensive, evidence-based review of modern VT management, beginning with the mechanisms that drive ventricular arrhythmias and progressing through pharmacologic and electrical therapies for refractory cases. Topics include antiarrhythmic selection, esmolol and sympathetic blockade, procedural sedation for recurrent cardioversion, vector-change defibrillation and DSED, electrolyte optimization, recognition of reversible causes, and the evolving role of ECMO and early transport to specialized cardiac centers. Using real-world cases and the latest literature, attendees will develop a practical framework for managing ventricular tachycardia from the first shock through advanced rescue therapies.
